Dear Participant,

Outward Bound programs are conducted indoors as well as outdoors, in a variety of weather conditions and can involve participants in activities such as rock-climbing, caving, ropes courses, hiking and a variety of physical and non-physical group tasks. Programs are adapted to meet the actual needs and abilities of the participants and participation in all activities is on a voluntary basis. 

In this context, may we ask you to complete this form? The information you provide will enable us to better adapt our program to your specific physical capabilities and to support you in respecting any physical limitations you may have.

This information will be treated confidentially. Any information you withhold will limit us in our ability to take your physical condition into account during our program
